The UNI ISO 45001: 2018 has brought some substantial changes to the framework of the previous standard in the field of safety management systems BS OHSAS 18001: 2007.
In particular, the need to put in place, within organizations, a valorisation of the operating experience and in particular of negative events is highlighted. These include accidents but also near-accidents, non-conformities and anomalies. Organizations are obliged to carry out evaluations to understand the root causes of these events in order to develop preventive and / or protective measures aimed at minimizing respectively the probability of a new occurrence and / or the expected consequences.
This article will briefly illustrate some methodologies and tools to support both the investigation phase and the subsequent phase of the “learning from experience”, providing an introduction to the use of techniques such as Root Cause Analysis (RCA), Tripod Beta, Barrier Failure Analysis , and BowTie. Methodologies that are also the basis of forensic engineering activities.
